2. Fill Out the Form

   • Complete all required fields, including student information, class selections, and emergency contact details.
      

3. Submit Your Form

   • Return the completed form in person to Ms. Cindy at the studio OR mail it to: 605 W. US Hwy 19E, Burnsville, NC 28714​
      

4. Call Ms. Cindy to arrange for your free trial class.
